Schedule set for school physicals

The Kansas State High School Activities Association requires all Peabody-Burns student athletes to have a current physical examination before participating in any type of athletic practice. This physical must be taken after May 1 in order to be valid for the upcoming 2013-2014 school year.

Physicals must be obtained on the form provided by the school district, and pages 1, 3, and 4 must be filled out and signed by the student-athlete and parent or guardian before a physical can be obtained. Forms may be found on the school’s website or picked up from the high school office.

The physicals will be given at Axtell Clinic, 700 Medical Center Drive, Suite 210 in Newton. The cost is $20 per physical and no appointment is needed. Dates for physicals are May 20 from 4 to 7 p.m., May 21 from 4 to 5:50 p.m., May 22 from 4 to 5:30 p.m., and May 23 from 4 to 7 p.m.

Fees are payable at the door and no insurance paperwork will be filled out. A parent or guardian also has the option of having an exam done by their family physician where all paperwork will be filed with the appropriate insurance carrier. If paperwork is needed, the student-athlete must make an appointment.

A student-athlete cannot participate in any type of school sponsored athletic activity until a signed physical is on file with the district. For more information, contact athletic director Ray Savage at (620) 983-2196, (620) 983-2817, or (620)382-4377.
